WHAT IS PROCTOLOGY, AND WHAT DOES IT TREAT?
Proctology is that part of Medicine that treats and studies the last part of the colon, namely the rectum and theanal orifice.
At the general academic level, these anatomical parts fall under the more general Colon-Proctology, which in turn is framed under the all-encompassing Gastroenterology.

WHO IS THE PROCTOLOGIST PHYSICIAN?
The Proctologist Physician is the health care specialist perfected in Proctology, able to intervene (including surgically) to resolve anorectal affections.

HOW IS THE PROCTOLOGICAL EXAMINATION PERFORMED?
The proctologic examination consists, like any other specialized medical examination, of four acts: history, diagnosis, treatment, and prognosis.
In the anamnesis phase, the proctology physician collects all information from the patient 's medical history, not only inherent to proctologic problems, but to all previous conditions in general, even any surgeries performed.
The purpose of the anamnesis is to provide the Proctologic Physician with all the information necessary to reconstruct the patient's medical history, since, very often, it is already in it that there is the possibileradiceof the proctologic problem.
The medical history is then followed by the clinical examination, which is essential to arrive at the diagnosis.
This clinical examination consists of three stages:
Abdominal examination
The patient'sabdomen is palpated with specific movements designed to establish the presence of air or inflammation known as colic chord.
The palpability of the bowel is an important signal, directing the physician to the right diagnosis;
Proctologic examination
The patient'sanal orifice and peri-anal area are evaluated clinically, either byvisual inspection or by digitalinspectionin which, gently and without any kind of pain, the Physician inspects the anus and the first part of the rectum with his finger, looking for lesions, malformations, any atypical or considered pathological features (such as, for example, failure of the sphincters to tighten in a resting condition);
Videoproctoscopy
The patient's rectum and anal canal are inspected using a special proctoscopic tube to which a high-definition video camera is attached, as well as the appropriate light source.
The examination is essential to assess the entire condition of the anus and rectum with extreme precision, making use of a magnification of about 20X, which is sufficient to inspect in detail even very small lesions (less than a millimeter).
The examination, which is noninvasive and painless, is performed without any preparation, and the patient can return to regular activities immediately, without waiting periods.
These three stages of the proctological examination, combined with the information retrieved in the medical history, lead the Proctology Physician to the diagnosis, which is followed by the appropriate treatment and prognosis.

IS THE PROCTOLOGICAL EXAMINATION PAINFUL?
Absolutely not: the proctological examination, if well performed by an experienced Proctology Physician, is not painful at all.
Even special cases, such as in the presence of prolapsed hemorrhoids or anal fissures, can be treated without pain for the patient, with appropriate and specific maneuvers (e.g., sphincter stretching).
Both digital exploration and videoproctoscopy are painless procedures and, contrary to what is commonly thought, not embarrassing.
Therefore, the patient should be calm and serene during the examination, and not delay it because of senseless fears or shameful conditions.
Delaying the proctologic examination out of fear or shame often only worsens the clinical course of many worsening conditions, such as hemorrhoidal prolapse.
It is therefore never a good idea, which moreover is even more foolish considering that the proctologic examination is absolutely painless when performed by an experienced Proctologist Surgeon.

CAN OTHER TESTS BE PRESCRIBED AT THE VISIT?
Of course: the Proctology Surgeon may often prescribe additional examinations if he or she feels there is a need for further investigation.
For example, an anorectal manometry may be prescribed to assess sphincter tone (useful in cases of suspected sphincter hypertone with a fissure ascertained by videoproctoscopy), or, for example, an endoanal ultrasound with a rotating probe, which is essential to establish the path of an anal fistula.
In general, any examination that the proctology physician deems indicated to reinforce or confirm the diagnosis may be prescribed.

Dr. Luisella Troyer is a Vascular Surgeon, specialising in Vascular Surgery and Proctology.
Since her training as a resident, Dr. Troyer has been interested in the study and treatment of proctological pathologies, especially haemorrhoids and haemorrhoid prolapses.
